The Problem
Acute sciatica during a 15km training run - sharp pain from lower back down left leg. Could not sit for more than 10 minutes. MRI showed L4-L5 disc herniation pressing on the nerve root. Orthopaedic surgeon suggested considering surgery if no improvement in 8 weeks.
The Treatment
Week 1-2: McKenzie directional exercises 6 times daily to centralise pain. Nerve glide exercises. Avoid sitting - standing desk at work. Week 3-6: Pain centralised to lower back only. Begin core stabilisation. Pool running for cardio. Week 7-12: Progressive return to running - walk/jog intervals. Week 13-20: Full running program, speed work, race-specific training.
The Outcome
Completed Ipoh International Run half marathon at month 5 - finishing time 2:05, only 8 minutes slower than pre-injury PB. Zero sciatic symptoms during or after the race. Repeat MRI at 6 months showed disc reabsorption. No surgery needed.
